Registration for new patients

Patients without a general practitioner service and have medical cards are welcome to register with Coolock Family Practice. Upon their initial visit, patients will have to fill out a patient registration form and sign a letter asking for their previous physician's medical records.

Medical care is accessible as of the registration date. For more information, kindly get in touch with reception. In addition to private patients, we provide a comprehensive spectrum of medical services to GMS patients who have a medical card or doctor visit card.
